In our center we use yet another spermatozoa retrieval method, MESA. This approach ensures spermatozoa retrieval in the so-called obstruction factor of male infertility, when spermatozoa are produced but cannot be naturally discharged from the testis because of obstruction on a certain level of the vas deferens. This is the highest level of microsurgery, making it possible to enhance the probability of getting good sperm-containing aspirate. In the reproduction center EmbryLife, such manipulations are performed by Mikhail Alekseyev, urologist/andrologist. Thanks to the cooperation with plastic surgeons and microsurgeons from GrandMed Plastic Surgery Clinic, the use of a surgical microscope and special microsurgical materials and instruments, the MESA can be conducted in the gentlest manner, with minimum injuries.

Spermatozoa retrieval technology

1 U/S guided aspirate retrieval from the testes, fluid retrieval from the epididymis, spermatocele or rete are performed under general anesthesia, using a biopsy micro-instrument (needle). Or a microscopic tissue fragment is micro-surgically retrieved under general anesthesia and with U/S control
2 Then, a laboratory retrieves spermatozoa from the material obtained as a result of the surgery or paracentesis. This tissue is moved to a nutrient solution, in which mature (even single) and immature spermatozoa are separated
3 Then, the most healthy and high-quality of the obtained spermatozoa are selected (using a microscope) to be used for ovum fertilization (ICSI)
4 Then, the embryos are cultivated in vitro and transferred into the uterus at the age of 3 to 5 days (IVF)
Important: Pregnancy is not affected by the method of spermatozoa retrieval; it will not differ from a normal pregnancy.

The TESA or PESA procedure takes 10 to 15 minutes. The MESA procedure takes about one hour and involves hospital admission for 3 to 4 hours. Neither of the procedures affects sexual potency or other male functions. The retrieved sperm is used for IVF. TESE or PESA (MESA) are rather frequent procedures. Especially in those who prefer not to get their sperm discharge function permanently restored by surgical methods.
