Ova quality and age

“Regardless of whether we want it or not, the average age of bearing the first child is becoming increasingly higher. It was age 25 in the past year. This is the alarm signal,” O. Golodets, Russian Deputy Prime Minister, said. Why is she so concerned with the age of women who decide to have their first child?

The reproductive chances for a woman are restricted by nature.

The principal factors for a successful pregnancy are:

  • the number of ova
  • quality of the ova, both of which are inevitably reduced each year. 

The probability of having a child reaches its maximum at the age of 22, then it falls by 50% by age 37, and is almost zero at the age of 45 – 47. At the same time, currently most of the young women, considering their financial and social status and, probably, just the current absence of a man who would be worth starting a family with, prefer not to hurry having their first child. However, despite the reasonableness of such behavior by the prospective mother, it should be remembered that a woman loses her ovarian reserve with each year, i.e., the number of follicles capable of further development decreases. Moreover, almost every woman acquires additional factors hindering pregnancy as time passes: consequences of inflammatory illnesses or surgery (not only gynecological), complicated abortion and endometriosis. 

Solutions for late motherhood problems

Currently there is a growing trend where young women not planning to get pregnant in the near future come to the IVF clinics for cryo-preservation of their ova. This allows them to store their ova for future fertilization. By freezing your ovum, you can become pregnant without a donor when you are sure of your personal and economic stability. Since the frozen ova do not age, the chances of getting pregnant using them in the future cycle will remain unchanged, regardless of the woman’s age. In other words, if the ova were frozen at age 25 years and the IVF procedure will be performed at age 40, the woman’s chances of getting pregnant will be the same as at age 25. Hormonal stimulation in such patients is conducted in the same manner as for others. The only difference in their cycle is that after the ova are taken, they are not fertilized, but are frozen using vitrification. 

Solutions for ovariectomy and oncology problems

Another, newer method for fertility preservation is ovarian tissue freezing. This service is also used by women who are to undergo chemotherapy during oncological illnesses, or ovariectomy. (Unfortunately, up to 8% cases of cancer are reported by women of reproductive age, and 70% of them want to have children after treatment).A biopsy involves removing an upper, about 1 mm thick (cortical) layer from the ovary: all the ova are right here. This layer is then cut and frozen for storage (it may be transplanted at any time). The ovarian tissue freezing and subsequent transplantation make it possible to completely regenerate the reproductive function and reverse menopause.  The menstrual cycle usually resumes by a woman three to six months after such transplantation. The ovarian tissue removal, freezing and transplantation technologies are rather sophisticated. Currently, there are only a few clinics in the world with a good reputation in this area. For example, Dr. Sherman Silber of St. Louis (United States) is the most experienced person in this field according to Dr. D. Dozortsev.
